Please note that where Intrinsic Analytics has been contracted to provide biochemical analytical testing services on behalf of a third party or third party employer, test results will only be provided to such third party or third party employer. If this arrangement is unacceptable to you, please do not use our office for such testing services.

Depending on the nature of services requested, it may be necessary to send your personal information/personal health information to another laboratory or third party testing facility for confirmatory or further testing. You will be advised of such transmission at or prior to the time your information is disclosed and your information, once transmitted will be subject to third party practices and policies - Intrinsic Analytics will not be responsible or liable for any uses made by such facilities.

To the extent Intrinsic Analytics uses third party service providers in connection with its business who may have access to personal information, Intrinsic Analytics uses contractual means to bind such third parties to its privacy obligations.

Retention and Safeguarding of Personal Information/Personal Health Information

Intrinsic Analytics retains personal information and personal health information on file in accordance with legal requirements.

Intrinsic Analytics has established security safeguards and practices to better protect personal information and personal health information under our custody and control; these safeguards include administrative, technical and physical safeguards that take into account the sensitivity of your personal information. We review our security safeguards on an ongoing basis and require an internal audit of our practices and safeguards at least every two years.

Destruction of Personal Information

When records or hardware are being destroyed or disposed of by Intrinsic Analytics, steps are taken to ensure that no personal information is released during the destruction or disposal process. All paper records to be disposed of by Intrinsic Analytics are shredded.

Access and Correction

Generally speaking, and excepting limited circumstances, individuals have a right to access their personal information and personal health information maintained by Intrinsic Analytics. An individual may access his or her records by request in writing. Should individuals have a concern about the accuracy or completeness of their personal information or personal health information, they may apply in writing to Intrinsic Analytics for the correction of same. Intrinsic Analytics shall review any such requests, and respond within the prescribed period.
